What Information Do You Gather About Me?
We collect from you only the personal information you voluntarily provide in order to deliver the best possible service. If you choose to process an online payment or reservation, we ask you to provide us with information necessary to process your order, which may include, but not be limited to, name, physical address, telephone number, facsimile number, email address, company name, title, credit card number, expiration date, and storage unit number. We collect your email address so that we can send you information about your transaction and its status. We may use your email address to inform you about current offers or events. We will never share, sell, or rent individual personal information with anyone without your advance permission or unless ordered by a court of law.
Sharing Your Personal Information
Manor Lake Management (encompassing the brands Horizon Living and Manor Lake) does not sell information to unrelated third parties. Manor Lake Management does send your contact and order information to the community that you selected for purposes of satisfying your request for a brochure or a tour.
Changes to This Online Privacy Policy
Manor Lake Management reserves the right, at its sole discretion, to change, modify, add, or remove portions of this Online Privacy Policy at any time. Accordingly, Manor Lake Management recommends that you revisit this Online Privacy Policy from time to time to ensure that you are aware of the current privacy practices. Your continued use of the website following any changes signifies your acceptance of these changes.
Manor Lake Management and Session Cookies
The Manor Lake Management website uses session cookies to enable making transactions on our site. A session cookie is a small file placed on your computer which acts as a unique identifier to keep track of the information you have entered. Without the session cookie, our site cannot track the information you are entering, and you will be unable to complete your transaction. Protected Health Information (PHI) is any information that identifies the past, present, or future physical or mental health of an individual and includes electronic, written, or verbal communications.
WHO WILL FOLLOW THIS NOTICE
This notice is followed by all Manor Lake Management staff (including those at Horizon Living and Manor Lake communities) and any independent and third-party entities affiliated with Manor Lake Management. We are committed to protecting your health information because we understand that your medical information is personal by:
- Following the terms of the notice that are currently in effect.
- Notifying you of our legal duties and privacy practices with respect to your PHI.
- Making sure that your PHI is kept private.
Pursuant to either patient authorization or applicable laws and regulations, your PHI can be disclosed to other parties for the following reasons:
- For Treatment: We may disclose PHI about you to healthcare personnel at Manor Lake Management or an independent and third-party entity who are involved in taking care of you. However, an authorization is required from you in order to release psychotherapy notes.
- For Payment: To bill your insurance company, or a third party for services provided to you. However, if you pay out of pocket in full for any service provided to you, you have the right to restrict the disclosure of certain PHI to a health plan.
- For Health Care Operations: To review our treatment and services and to evaluate the performance of our staff in caring for you or to disclose information to nurses, doctors, technicians, nursing staff, and other personnel for review and learning purposes.
- Appointment Reminders and Follow-up Calls: We may leave a message in an answering service or your answering machine.
- Research: Under certain circumstances, we may use and/or disclose PHI about you for medical research purposes.
- As Required By Law: Your PHI will be disclosed when required to do so by federal, state, or local law.
